Leading Senior Constable Ian Wallace said the accident occurred at 12.30pm on Tuesday, when a truck was taking a bend on Porcupine Ridge Road, near Daylesford.
"It looks like the driver has taken the corner too wide, tried to correct, and lost control," he said.
"The bike was travelling the other way and taken evasive action because the truck was on the wrong side of the road.
"It was then they collided and the truck rolled."
Ambulance Victoria spokeswoman Gabrielle Degenhardt said the motorbike rider, in his 60s, suffered serious injuries to his left leg and was airlifted to The Alfred hospital.
The truck driver, in his 40s, suffered a laceration to his arm and was transported to Ballarat Base Hospital.
There were no passengers in either vehicle.
"It's absolutely lucky everyone survived," Leading Senior Constable Wallace said.
"The message is to take care on country roads, especially around the bends.
"If you're not sure of the bend, just slow down."
Police are continuing their investigations.
